
Practice Closure

I have made the decision to close my private practice
effective December 31, 2025.
Although I initially hoped to continue operating,
the administrative demands of outpatient psychiatric care
have increased significantly and
are no longer sustainable for me as a solo provider.

If you were previously seen at Silver State Counseling/Life Quest, after July 1, 2025, please contact them directly to coordinate continuation of care.

I strongly encourage all patients to contact your
insurance plan’s member services
for assistance in identifying
in-network psychiatric providers.

RELEASE OF INFORMATION
RECORDS ACCESS AND RELEASE POLICY

HIPAA (Federal Law)
Record requests are processed pursuant to the patient’s right of access under 45 CFR § 164.524. The practice will respond within 30 days of receipt of a valid written request. If additional time is required, written notice will be provided as permitted by federal law.

Nevada Law
Nevada law distinguishes between inspection of records and furnishing copies. Under Nevada Revised Statutes (NRS) 629.061, inspection permits review of records within 10 working days of a valid request but does not constitute transfer of possession.

Inspection
As the practice no longer maintains a physical office location, in-person inspection is not available. Records may be furnished as copies upon receipt of a valid written request.

Transmittal of Copies
Furnishing copies requires a complete, signed, and dated written authorization and is governed by NRS 629.061, NRS 629.062, and applicable federal law.

FEE SCHEDULE
Fees are assessed in accordance with Nevada and federal law.

Paper Copies (NRS 629.061)
• $0.60 per page (pages 1–20)
• $0.50 per page (pages 21–50)
• $0.20 per page (pages 51+)
• Actual postage, if mailed

Electronic Copies (45 CFR § 164.524)
• Flat fee of $6.50
• Cost of electronic media, if applicable

No retrieval or administrative fees are charged.

Payment
An invoice for applicable fees will be provided to the requester. Payment is required prior to furnishing copies.

Provider-to-Provider Transmission
There is no charge for records transmitted directly to another licensed health care provider for purposes of continuity of care. The receiving provider’s complete contact information must be provided at the time of request.

Identification
For electronic or mailed requests, a copy of a valid government-issued photo identification is required to verify the identity of the patient or authorized representative prior to release of records.

Authorization Requirements
Processing of records requests requires a complete, signed, and dated authorization containing all required elements under federal and state law. Requests that are incomplete or missing required information will be returned for correction prior to processing.
